Our office is located at 2301 Ohio Drive, Suite 200. Use the south entrance, the door with the awning. Go up the stairs (or elevator) to the second floor. If you use the stairs, make a left at the stairs and we are on the left. If you use the elevator head straight and we are on the left. The restroom is down the hall from our office, make a right at the phone equipment closet and restroom is on the left. We also see people virtually, on phone or telemed video. 

We do not take insurance for appointments but some of our testing is covered by insurance. Reimbursements are not guaranteed and are between you and your insurance or medishare program. We DO NOT provide Superbills. If you require additional documentation from our office for submission to your insurance company or medishare, there is an additional charge for administrative time. Our already discounted cash prices do not include billing insurance or medishare paperwork involvement.

We do take HSA cards and FSA although some require a letter from our office to get your charges approved which we are happy to provide. There is no fee for a standard letter for verification but there is a charge if you require customized forms.

When you schedule your first appointment with us, you automatically generate a patient portal. You can access immediately. In the document section you can add previous labwork if you want. The conversation center is for specific types of questions or comments related to your most recent visit. Do NOT use the portal to order supplements, or scheduling. If you do not know how to use the portal please call the front desk for information.

The intake form comes to you either in a text or email as a link to complete. You must click SUBMIT in order for us to receive the document. If you were not able to complete the form prior to your appointment, PLEASE come 20 minutes earlier than your appointment time to complete the intake form. 

The appointment is always in your name, the parent/adult because the appointment is with you regarding your child, but the intake form is completed with your child’s information. Your child is NOT needed for the appointment, and it is preferred that you have uninterrupted time for your appointment.

If it has been longer than two years since your last visit, you need to set up again as a new but with a significant discount so please call our office to have the discount applied. If it has been less than a year schedule the extended follow up to make sure we cover any and all changes in your health.  

We sell the supplements we recommend. In the event we have recommended a special product we do not stock we will either order it special for you or tell you where to purchase. We can also help you use the current supplements you have on hand if they are or will be appropriate to you going forward.

We work with any diagnosed condition although you do not have to have a diagnosed condition to work with us, in fact many chronic issues do not have a typical diagnosis. If you need an advanced diagnosis our nurse practitioner will do this for you.

We specializes in hormones. We offer both traditional and functional hormone testing. Hormones is one of our specialties.

No, lab work does not need to be completed prior to your appointment. If you already have lab work completed within the past year, please add to your portal, but if you have not had any traditional lab work done in the past 1-2 years we will take care of that for you in our office.

Yes, our Nurse Practitioner can write any prescription for anything needed.
