Our office is located at 2301 Ohio Drive, Suite 200. Use the south entrance, the door with the awning. Go up the stairs (or elevator) to the second floor. If you use the stairs, make a left at the stairs and we are on the left. If you use the elevator head straight and we are on the left. The restroom is down the hall from our office, make a right at the phone equipment closet and restroom is on the left. We also see people virtually, on phone or telemed video. 

We do not take insurance for appointments but some of our testing is covered by insurance. Reimbursements are not guaranteed and are between you and your insurance or medishare program. We DO NOT provide Superbills at this time. If you require additional documentation from our office for submission to your insurance company or medishare, there may be an additional charge for administrative time depending on the extensive nature of some forms. Our already discounted cash prices do not include billing insurance or medishare paperwork involvement. We also accept Afterpay if you need a payment plan.

We are passionate about the plans we create to help individuals take charge of their health. We want each person who takes the first step and commits to their initial appointment to invest in their health and wellness. Take this journey with us, where the end goal in the first year is to be weaned down to a few supplements and a maintenance routine of one or two times a year follow-ups (excluding if a person is on Rx with a more frequent refill schedule or hormone therapy). Our initial appointment is $344 and requires prepayment as we begin working on setting you up in our office with a patient portal and more. The first appointment does not include any lab work. Traditional lab work will be billed through your insurance if you have insurance. Functional testing is not considered diagnostic testing and therefore is not covered by insurance. To assist in this process we offer functional tests at discounted pricing. Functional tests are optional but incredibly helpful for doing the deep dive into your health and getting information you cannot get in diagnostic testing. Functional testing at a wholesale price can range anywhere from $300-$650 per test kit. Our lab reviews range from $99 to $250 depending on what tests are reviewed. Most of our followups are 30 minutes for $69-89 depending on the additional time we spend along with your followup. We do prorate our time and if we feel appropriate, we adjust the payment accordingly. All our services incur a charge, whether it be something we do for you outside of an appointment or within an appointment. All of our services are paid at the time of service with no refunds available for our time. We also accept Afterpay if you need a payment plan. Remember, the first year incurs the most charges as this is the process to get the most comprehensive baseline to resolve your acute issues and guide you to a solid maintenance without regressing back to the beginning.

We do take HSA cards and FSA although some require a letter from our office to get your charges approved which we are happy to provide. There is no fee for a standard letter for verification but there is a charge if you require customized forms. We also accept Afterpay if you need a payment plan.

When you schedule your first appointment with us, we work immediately to set up your patient portal. You can access your portal from the top right corner of the website using the email you used to set up the appointment. In the document section you can add previous labwork and we will add your documents after the appointments. The conversation center is for specific types of questions or comments related to your most recent visit and may incur a charge. Do NOT use the portal to order supplements, or scheduling. If you do not know how to use the portal please call the front desk for information.

The intake form comes to you either in a text or email as a link to complete. You must click SUBMIT in order for us to receive the document. If you were not able to complete the form prior to your appointment, PLEASE come 20 minutes earlier than your appointment time to complete the intake form. 

We see ages 12 and up. If you schedule the initial appointment online please do so in the parent name, and complete the intake form in the child’s name. The first appointment is for the parent/s and is best completed if the child is not present, unless age 16 and over then the child is required to be present. If your child is 18 and over, your child will be required to sign a consent form to allow us to speak with you on matters of the child’s health.

If you want to ask Laura a question, and you are an existing client currently on a health journey, you can message Laura privately in the conversation center of the portal. From there someone in our staff will reach back out to you depending on the question. If appropriate for Laura to answer, she will get back to you. In most cases you might be asked to schedule an appointment. We are getting ready to launch our membership program, and our VIP Membership will have an Ask Laura community chat room.

If it has been longer than two years since your last visit, please call our office to see what appointment you need. If it has been less than a year or you are due for your one year please schedule a regular 30 min follow up. 

We sell the supplements we recommend. In the event we have recommended a special product we do not stock we will either order it special for you or tell you where to purchase. We can also help you use the current supplements you have on hand if they are or will be appropriate to you going forward.

We work with any diagnosed condition. If you need a diagnosis, our Nurse Practitioner can give you a diagnosis or advanced diagnosis and/or referral for imaging or specialists if appropriate to your care. You do not have to have a diagnosed condition to work with us, in fact many chronic issues do not have a typical diagnosis.

We specialize in hormones and we are very proud of how different and comprehensive we are with hormones. No one we know of in either functional or diagnostic arenas approach hormones the way we do, utilizing both saliva and blood, both traditional and functional tests to get the most comprehensive look at not only your hormones, but how your hormones are behaving in your entire body. Limited testing is one of the downfalls of current hormone care. Experience a better way of doing your hormones with us.

We want you to have bloodwork done in our office with our Nurse Practitioner. If you already have lab work completed within the past year, please add to your portal, but if you have not had any traditional lab work done in the past 1-2 years please wait until you have your first appointment. We prefer to determine what labs we would like you to do, so please wait until you have met with us.

Yes, our Nurse Practitioner can write any prescription for anything needed. 

Our Nurse Practitioner is here to help you navigate your prescriptions and work toward a safe transition if applicable to your health.

You refill your supplements either on the website under the Buy Supplement Section or from our app called Kopec Functional Wellness. Please make sure you have reviewed your care plan and are following your refill schedule. We do not want you to refill something you do not need. If you are due for blood work in the next few days then hold off on your refills that time out with getting your blood work repeated.

You refill your Rx prescriptions with the Nurse Practitioner according to the refill/blood work schedule you are on. Prescription refills are different from supplement refills and require the Nurse Practitioner to determine your refill and blood work schedule. We do not make exceptions to this for your own safety. When in doubt, call or schedule a followup with our NP.
