Office Policies

We believe clear expectations help create a positive, respectful experience for everyone. These policies allow us to provide focused, unrushed care while keeping our services accessible and affordable.

Appointments
All appointments may be scheduled:
Directly through our website
By calling or texting our office at 972-942-4039
Appointment availability varies by practitioner. Telehealth and Phone appointments are available. Please select the provider or department you wish to schedule with to view available times.
Appointments are scheduled in Central Time (CST). You may sync appointments to your personal calendar, which will automatically adjust to your local time zone.
 
Cancellations & No-Shows
We kindly ask for at least 24/48 business hours’ notice if you need to cancel or reschedule. Late or same day cancellations and no-shows may be subject to a fee.
 
Payments
Payment is due at the time of service. We accept all credit cards, debit/ACH, Venmo, Zelle, HSA and FSA cards. 

Insurance & Superbills
We bill diagnostic bloodwork directly to your insurance provider. Superbills are available upon request for eligible services and must be requested within 90 days of your appointment. Insurance reimbursement is not guaranteed. If your insurance or medical sharing program requires additional documentation from us an additional fee may apply for the time required to complete this work. 

Lab Services
Our primary lab partner is Quest. Please let our team know as soon as your blood is drawn if you need an alternative lab option or prefer cash-pay. 
 
Communication
For scheduling, general questions or ordering product, please contact us by phone or text at 972-942-4039. We respond within 24–48 business hours whenever possible. Text messages are not HIPAA-compliant and should not be used to discuss personal health information. For any health-related questions or clinical communication, please use the patient portal.
The messaging feature within the patient portal is monitored and may incur time-based fees, similar to appointments, when clinical review or provider response is required.

Prescription Refills
Please allow 48–72 hours for prescription refill requests following your appointment. Prescription refill requests may not be made in the patient portal without a recent appointment and necessary lab work.
 
Refunds and Returns
Unopened product may be returned for a full refund within 2 weeks of purchase.  Initial appointments, lab work and test kits are nonrefundable. 
 
Results Disclaimer
Individual results may vary, and some services require multiple sessions for optimal outcomes.
 
Right to Refuse Service
Optim8 Health & Wellness is committed to maintaining a safe, respectful, and professional environment for patients and staff. We reserve the right to refuse services when necessary to uphold these standards. In such cases, payments made are non-refundable.
